MEMBERSHIP

You have the option to provide demographic information (such as disability, and preferred accessible formats) to us. We encourage you to submit this information so we can refer you to appropriate programs at CCDC, and provide materials in accessible format. Joining CCDC is completely voluntary and you therefore have a choice whether or not to disclose this information. The reason we ask for information such as race is so we can hold ourselves accountable for assuring we are serving all communities. The only way we can assure if our outreach is effective and inclusive is if we can review who is and who is not joining our organization and involving themselves in our programs.

You will receive, from time to time, letters, newsletters, and requests for donations via postal mail. If you request, you will also be subscribed to our CCDC e-mail alert list.

SURVEYS

From time-to-time we may provide you the opportunity to participate in a survey on our site. If you participate, we will request certain personally identifiable information from you. Participation in these surveys is completely voluntary and you therefore have a choice whether or not to disclose this information. The requested information typically includes, without limitation, contact information (such
as name and address), and demographic information (such as zip code).

Whenever we create a survey, we explain how and why the data is collected, and how it will be used. We may use a third party service provider to conduct these surveys; that company will be prohibited from using our users’ personally identifiable information for any other purpose. We will not share the personally identifiable information you provide through a survey with other third parties unless we give you prior notice and choice.

USES OF INFORMATION

We use the information collected to process your membership, to provide you with the information you requested, and to provide assistance to you if you request. We will not share any personally identifiable information with anyone outside the organization without your express permission. This site is created primarily for the benefit of our members, as well as people seeking disability related information.

We do use aggregate (i.e., non-personally identifying) information about our members for purposes of promoting CCDC goals and objectives, for media purposes, and for fundraising.

ELECTRONIC NEWSLETTERS AND ALERTS

If you wish to subscribe to our electronic newsletters and alerts, we will use your name and email address to send this information to you. Out of respect for your privacy, we provide you a way to unsubscribe. Please see the “Choice and Opt-out” section below.

INFORMATION SHARING DISCLOSURE – AGGREGATE INFORMATION (NON-PERSONALLY IDENTIFIABLE)

We share aggregated demographic information about our members with our allies, media, and funders. This information does not identify individual members. We will not use any personally identifiable information about you without your express permission.

We do not link aggregate member data with personally identifiable information.

PERSONALLY IDENTIFIABLE INFORMATION

We DO NOT share personally identifiable information with third parties without your express permission. We do not sell our address list to companies or other organizations. From time to time, we allow outside organizations to pay CCDC to mail or email information to you, but we will never release your personally identifiable information to a third party without your express permission.

LEGAL DISCLAIMER

We reserve the right to disclose your personally identifiable information as required by law and when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, court order, or legal process served on our website.

LOG FILES

As is true of most Web sites, we gather certain information automatically and store it in log files. We use this information, which does not identify individual users, to analyze trends, to administer the site, to track users’ movements around the site and to gather demographic information about our user base as a whole.
