Set the stage not only for needed protections in managed care (which was then the future of all health care) but for the importance of including the affected people in decision making which was a new experience for Colorado government

Due to influence of CCDC Medicaid Advisory Committee for People with
Disabilities Formed-subgroup of this was the disability working group.

People with disabilities involved in direct policy making. Disability working group model was written up as national study about way to appropriately include the disabled.

CCDC leads passage of the law enacting the Consumer Directed Attendant Support Program. Bill also included medically correctible pilot (since defunded). We
were also able to expand the number of children served in the children HCBS program.

Fort Collins, access improvements in many communities throughout Colorado and
inclusion of disability advocates on local boards and committees.

CCDC involved in creating fast track pilot program to help expedite community based long term care eligibility. CCDC also did survey and produced report on what service gaps led currently institutionalized people into facility based care.

This was beginning of a commitment by the state to actively move people out of institutions. Subsequent actions led to a state funded nursing facility transition program.

CCDC obtains spousal impoverishment protection for HCBS clients (previously this protection was only afforded to people in nursing facilities)

CCDC enters into five year consent decree with RTD. Consent decree has been and continues to be seen as a national model of best practice in terms of transit ADA compliance

CCDC celebrates 10 years of the ADA and of CCDC reaching over 500 people at our celebrations

As a result of the Olmstead Decision CCDC and ADAPT are able to help the state obtain millions of dollars in system change grants to make improvements in HCBS service delivery

Relevance
Olmstead was a major Supreme Court decision that reinforced the state obligation to make reasonable modifications in policy if needed to serve individuals in the most integrated setting.

Achievement
CCDC members who have lost custody of children due to children having mental health needs form coalition and successfully pass a law that prohibits social services to change parents with dependency and neglect simply because the child needs mental health care.

Achievement
CCDC successfully staves off budget cuts during economic crisis and in coalition with ADAPT gets the state Medicaid agency to make a major change in how they determine eligibility for long term care. Development of ULTC 100.2 screening tool and true commitment to consumer direction

This was a major shift in attitude at the Medicaid agency and a great example of the combination of direct action (done by ADAPT in a 14 day camp out in front of the HCPF office) and policy guidance from CCDC worked to make the system better and more fair to everyone. Previous system over - emphasized physical disabilities at the expense of people with mental and cognitive disabilities

Achievement
CCDC helps to implement changes from 2002 and Consumer Direction finally begins as a service.

Relevance
CCDC had been actively working on building the infrastructure for consumer direction for seven years.

Achievement
CCDC did a tour around the state educating people on how the state budget works, developed documents on how to understand the budget process. CCDC also developed a self assessment for long term care services.

As a result all FREX buses have lifts and necessary policy changes were made

CCDC began dialogue with providers and the state regarding creation of a disability care coordination program

Legislation was passed in 2006 and the program opened in 2008 and is now serving clients. The program is the Colorado Alliance for Health and Independence. CCDC and Family Voices are the corporate members responsible for appointment of the board.

Consumer Direction becomes formal part of Medicaid – law passed to make it a service delivery model in all waivers.

CCDC has assistance available to help people access long term care in more than 60 Colorado Counties

Achievement
CCDC provided training around the state on the new Medicare Part D benefit. As a result of our training and outreach many people with disabilities were ready and did not have disruption in services. CCDC worked closely with the local CMS office to resolve transition problems for individuals.

Relevance
CCDC was the only organization providing information and assistance on the impact of Medicare Part D benefit for young disabled who are dually eligible for Medicaid and Medicare.

Achievement
CCDC is part of largest and most comprehensive ADA settlement ever when we settled with K-Mart

The settlement came a day after the 16th anniversary of the ADA and signaled a shift nationally that the ADA was to be taken seriously and is here to stay

CCDC took the lead in making sure that disability was considered in all aspects of planning for the National Democratic Convention.

As a result of our advocacy an ADA coordinator was hired and was part of the Host Committee. The areas of the convention controlled by the Host Committee (as opposed to the DNC) had full access and this was the most accessible national convention (either party) in US History.

CCDC settles lawsuit with Ellie Caulkin’s Opera house completing full accessibility in all of Denver cultural venues

CCDC had previously successfully improved access at Red Rocks, Coors Field, the Pepsi Center and Fiddlers Green

CCDC settles with all Over The Road Bus Companies operating in Colorado to assure full ADA compliance

CCDC works with foundations, state agency heads, and other disability organizations to create awareness of the need for a statewide application assistance program to help people access health care and financial security.

There is now a large coalition working on development – there are several components including one focusing on homeless individuals led by the Coalition of Homeless, one component to tie pieces together and one to look at the 24 month gap while people wait for Medicare coverage
